When a skill is old or wrong, escalate that to human after the task is done.

Skill scripts must be cross-platform Node `.mjs` by default; use other runtimes only for domain-specific reasons.
Scripted skills must include `package.json` with `npm test` running `node --test` over their tests.

After refactoring a skill, run `pi --print` with a harmless real-world prompt that exercises that skill.
Use read-only actions or isolated temp creations only; do not modify existing external data.
